What Do I Need To Bring?

What to expect at your first visit:
When you arrive, the front desk will provide you with paperwork to fill out. When you have completed your paperwork, you will meet with VICRC personnel to enter your information into our computer system, VESTA. Once that is complete, you can receive services the same day. If you are missing one or more of the required documents, the interviewer will ask you to bring it for your next visit. 

First Visit

 For your first visit, we require:

Proof of address with visible date within the past 90 days for ALL adult household members (see list of acceptable documents)

Photo identification for all adults

Social Security card for each household member

Returning Visits

For regular visits, we require:

Proof of address (see list) with visible date within the past 90 days for ALL adult household members

Photo identification for the head of household

Seasonal Programs

For all of our seasonal programs, we require:

Proof of address (see list) with visible date within the past 90 days 

Photo identification for the head of household

-Social security cards for all children in the program (we can also accept health insurance cards OR
a document from a school with the child’s name and address, dated within 90 days, to verify their residence with you)


If you are missing the required documents, you may be denied services. We will do our best to work with your unique situation. If we cannot provide services, we invite you to return when you have required documents.

VICRC Proof of Address Policy

VICRC requires that every adult in a household verify they live in our service area at every visit when receiving services. Acceptable documents (see list) must be shown at time of service and dated within 90 days of the visit. Some programs may require proof of residency for youth.

If proof of address is not provided, the adult without proof will be determined ineligible for service until the following service visit and only served at the following visit if proof of address is provided.

Acceptable Documents for Adults (age 18 or older)

Documents must display the person’s name & address and dated within 90 days of visit.

  • LEASE, RENT RECEIPT, OR PROOF OF HOME OWNERSHIP
  • UTILITY BILL (FOR EXAMPLE: ELECTRIC, GAS, PHONE, WATER, SEWER, CABLE, SATELLITE, ETC.)
  • OTHER MAIL, IF POSTMARKED & DATED
  • BANK STATEMENT
  • PAYCHECK STATEMENT/PAY STUB, W-2, OR 1099
  • CERTIFIED COPY OF COURT ORDER
  • MILITARY DISCHARGE SEPARATION PAPERS
  • SCHOOL RECORD OR TRANSCRIPT
  • CHILD SUPPORT STATEMENT or ACCOUNT SUMMARY
  • COURT ORDER OF PROBATION, PAROLE, or MANDATORY RELEASE
  • PUBLIC ASSISTANCE BENEFITS STATEMENT
  • OFFENDER RELEASE IDENTIFICATION CARD

Identification will also be required at time of service. Driver’s license is not an accepted proof of address document.

VICRC cannot send mail on behalf of any client. If a person does not have access to any of the above acceptable documents, we recommend that a friend, family member, neighbor, etc. mail an envelope through US mail. The mail must be dated and addressed to the adult in need of proof of address.
